We are currently seeking compassionate and skilled Continuous Care LPNs and RNs to join our dedicated team. These positions are crucial in providing hands-on care, comfort, and support to patients and families during critical times-where your presence makes all the difference.
About the Role
As a Continuous Care Nurse, you will:
Deliver direct skilled bedside nursing care to hospice patients
Support patients experiencing acute symptoms that require extended care hours
Document interventions, medications, and care delivery accurately and timely
Work under the guidance of the RN Case Manager to ensure the care plan is followed
Provide compassionate, patient-centered support aligned with the hospice philosophy
Qualifications
Active New Jersey LPN or RN license
Minimum 1 year of clinical experience (hospice experience a plus)
Understanding of the physical, emotional, and spiritual needs of terminally ill patients
Strong critical thinking, communication, and emotional intelligence
Ability to work independently and collaboratively in a fast-paced environment
